When downloading the image for R80.20, we have the following options.
i would like to understand use of item 1 below , what is the difference of it compared with item 5 and 6.

The "3.10" information within the first column is referencing to the same image with the only difference that the "new" kernel version - 3.10 - is being instead used of the old on in the previous version.
More information regarding this change can be read here.
This release introduces the new 3.10 kernel for Security Gateways. It extends support for new platforms and offers a significant performance improvement in cloud environments.
Check Point R80.30 with Gaia 3.10 is now available. For more information, refer to sk152652.
For Security Management Server, the new 3.10 kernel is already part of Check Point Main-Train release starting from R80.20, and is supported on all Security Management platforms (see R80.20 Release Notes).
Regarding the other two options:
R80.20 Fresh Install and Upgrade for Security Gateway and Standalone
=> this is the CPuse package which allows you to upgrade or freshly install an already configured and running security gateway - therefore the .tgz file ending
R80.20 Gaia Fresh Install for Security Gateway and Standalone
=> this simply is an .iso file which allows you to install and set up a new, fresh, security gateway which previously did not have any CP software installed

Thanks. as for the R80.20 Gaia Fresh Install for Security Gateway and Standalone .ISO image when we install it as a VM, it can either be installed as a Gateway or Secure Management Server or as a Standalone with both g/w adn mgmt server on one VM.
is that correct understanding?
If this is the case why are there separate .ISO files for Gateway available for download and a separate .ISO file available for mgmt server.
Appreciate you can clarify.

1) Install ISO with 3.10 kernel for HP DL 360/380 G10 Open Server/ Cloud and more. See Check Point R80.20 with Gaia 3.10 for CloudGuard and Open Server Security Gateways ==> Install via DVD or ISOmorphic tool
2) Dual install ISO R80.10/R80,20 only for 6000 appliance. ==> Install via DVD or ISOmorphic tool
3) Build 166 - ISOmorphic - tool for creating a bootable SecurePlatform/Gaia flash device (USB stick with boot ISO boot image from point 1, 2 and 6)
4) Build 168 ISOmorphic for 3.10 kernel ISO's and R80.30 - tool for creating a bootable SecurePlatform/Gaia flash device (USB stick with boot ISO boot image from point 1, 2 and 6)
5) Install and Update tgz file for a CPUSE update via CLI or Web GUI on a running system. ==> Install or upgrade via CPUSE see Check Point Upgrade Service Engine (CPUSE) - Gaia Deployment Agent
6) Install ISO with 2.6 kernel for all otter appliances and open server ==> Install via DVD or ISOmorphic tool

If yes, why do we also have separate .ISO files for Gateway and Management server.

1 Basically is a Non Mainstream Release that is based on the 3.10 Kernel rather then 2.6.
Is basically for a limited number of OpenServers, ie Dell, HP
Tells more about the R80.20 3.10 release.
At the moment there is limited support Gateway Level for 3.10 releases even at R80.30.
If want to ensure get the correct image then follow the upgrade wizard
