checkpoint QoS on Voice traffic

I would like to create a new  QoS rule for Voice traffic at all of my locations. 

Does anyone have a sample policy they could share so I could see what that might look like when creating on the SMS with 80.40?

PS - all of the gateways in our ENV are SMB 1430's (77.20xx) and 1550's (80.20.x). Not sure if that makes any difference or not.
