Is App Control/URL Filtering enabled?
Might need to get the TAC involved.

Thank you @PhoneBoy for your replay,
App Control/URL Filtering is enabled.
I tired to disable it and enable it again but always the same result.

Look at this:
Have you NGTP subscription? Today I'm opening ticket, because application and URL filtering also doesn't work. I suppose that it'll be also licencing problem. Anybody know what should work with NGTP subscription?

APCL / URLF are covered by NGTP.
I believe the only SMB feature not covered by NGTP is Threat Extraction.
